Civil Rights Notice to Patients
CARD complies with all applicable Federal civil rights laws and does not discriminate on the basis of race, color, disability, sex, national origin or age. CARD shall not exclude or treat anyone differently due to race, color, disability, sex, national origin or age.
ASSISTANCE FOR DISABILITIES
CARD provides aids and services free of charge to our patients and their families who have disabilities in order to communicate effectively with us. This can include the following:
- Qualified Sign Language interpreters and
- Written information in other formats (large print, Braille, audio, accessible electronic formats etc).
ASSISTANCE FOR LIMITED ENGLISH PROFICIENCY (LEP)
CARD provides language services free of charge to our patients and their families whose primary language is not English or those who otherwise have Limited English Proficiency (LEP). Such services may include:
- Qualified interpreters or
- Information written in other languages
COMPLAINT PROCEDURE FOR PATIENTS
If you feel have been discriminated against in any way or you were not properly provided the above services, you may file a grievance or complaint.
You may file an informal complaint with CARD by contacting our Compliance Supervisor at the below information or by following up with the Operations Manager at your local clinic. You can file a grievance in person, or by calling 844-995-0384, or by using the Compliance portal.
